we have signed Business Associate Agreements (“BAAs”) with all third-party services that may

4. How We Share Your Information
We only share your information with parties involved in your care or required to deliver our
services. These may include:
- Treating providers, such as oral surgeons, orthodontists, periodontists, endodontists,
or other specialists involved in your treatment plan - Dental laboratories that fabricate restorations such as veneers, crowns, or aligners on
our prescription - Insurance companies and dental benefits administrators, for the purpose of verifying
coverage and processing claims - Payment processors, to handle payments for services rendered
- Practice management and technology vendors under signed Business Associate
Agreements, including our electronic records system, secure email, and HIPAA-
compliant form provider - Legal authorities, when required by law, subpoena, court order, or to protect the safety
of our patients and staff
We do not share patient information with advertisers or data brokers.
5. Data Retention
We retain patient records in our secure electronic practice management system in accordance
with Florida law, which requires dental records to be maintained for a minimum of seven (7)
years from the patient’s last visit, or longer when required by law or sound clinical practice.
Website form submissions that do not result in patient relationships are retained for a
reasonable period to allow for follow-up communication and then deleted or de-identified.
6. Cookies and Tracking Technologies
Our website uses cookies and similar technologies to improve functionality, analyze site traffic,
